Tadalafil Clinical Studies

A study was conducted by Michael Nelson and his counterparts termed as tadalafil-sensitive impairment in muscle blood flow in Duchenne Muscular Dystrophy (DMD). This study focused on tadalafil to find out whether it causes a blood flow blockage in people with DMD a condition caused by the mutation of the dystrophin gene. Those with this condition experience wasting or shedding of muscles and there is no treatment for it apart from using corticosteroids.

Patients with DMD do not have the muscle protein “”dystrophin” and this leads to an impairment of nNOS and its process of targeting sarcolemma where the muscles get nitric oxide.

 

This impairment makes the dystrophic skeletal muscles fail to show reflexes induced by certain exercises. This links the pathogenesis of DMD with a theoretical vascular element. In this study, data was forwarded using the second phenotype and it shown that blood flow in boys with DMD when measured by using Doppler ultrasound of brachial artery did not go up during a mild exercise such as a hand grip.

 

The role of tadalafil was clearly laid out during this study when it was used an agent that removed hyperaemia in those suffering from DMD in a way of dose dependence. This study showed the effectiveness of PDE5 therapy as an important way to treat muscular disease.

 

Another study was conducted by Xu Ziqiang and counterparts to unearth the effects of tadalafil on arteriosclerosis of a graft in rats. Tadalafil can be used to regain the functions of the endothelium and as a treatment of arteriosclerosis but no studies had been conducted to know how it affects the arteriosclerosis developed on a grafted tissue.

 

For this study, male Brow Norway rats were picked to provide aorta grafts to the male Lewis rats. The rats were placed into 3 groups:

• Group 2, the rats were given low doses of tadalafil i.e. 0.5 mg per kg as a daily dosage
• Group 3, the rats received high doses of tadalafil i.e. 1 mg per kg every day.

8 weeks after the transplant, the graft was removed and studied using various tests. To test the level of cGMP, the ELISA process was used and the researchers found that the use of tadalafil led to the decrease of aorta thickness. Tadalafil also led to a concentration increase of cGMP and the cGMP-dependant, kinase enzyme in the rats.

 

How Tadalafil Works

Tadalafil works by stopping the release of the enzyme phosphodiesterase 5 commonly referred to as PDE5. This enzyme breaks down cGMP found in the corpus cavernosum in the penile region. When a man is sexually stimulated, there is an increased flow of blood towards the penis as the arteries are dilated. The dilation of the arteries occurs after the production of Nitric Oxide in the penile tissue. When the Nitric Oxide is produced, more cGMP is released in the corpus cavernosum leading to a relaxation of the penile muscles. PDE5 stops the accumulation of cGMP and its effects and tadalafil inhibits PDE5 to treat ED.
